To fill out and download the KOPFC membership forms, you need the Free Adobe Acrobat Reader.
Directions:
1. Click on the appropriate .pdf form
2. Fill out the form (use 'tab' or mouse to move to the next field and 'space bar' or mouse to assign check marks).
3. Print the form by clicking the printer icon in Adobe Acrobat
4. Bring the completed form along with proof of residency (if applicable) to KOPFC.
